US Annual Application Checklist
Thank you for your interest in The Surf RV Resort. It is the policy of our Resort that records must accurately reflect the name and contact information of the person/persons who will actually be occupying a site.
- A signed application with all questions and blanks completed
- $50 non-refundable processing fee per application paid via check or credit card
- Proof of Income: Bank Statement showing record of Social Security Income, pension income, Direct Deposit or disability income. W2 from prior year or current pay stubs
- US Patriot Act form completed – Must provide Photo ID, such as drivers license, State ID, Military ID, US Passport, or Residence I-551 (Green Card) with your name, address, and date of birth. Provide a Credit Card, insurance card, Medicare/Medicard card, birth certificate, or social security card
This resort reserves the right to refuse application to any person or proposed occupant if they have been convicted of a criminal offense.
Upon approval and reservation the first month’s site fee must be paid and a valid credit card on file for future payments. A Rate and Use Disclosure is required at the time of Reservation. All applications and supporting documents can be returned to the resort office either in person, or via email to Info@thesurfrvresort.com
